- PSA marriage contract for married female applicants unless the maiden name is preferred
- Transcript of records with CHED Special Order number where applicable, a scanned picture, and the remark For Board Examination Purposes

Good moral character and fee entry
PRC lists one certificate of good moral character from a barangay, church, school, or employer.
The same entry lists P900 for a complete examination and P450 for a conditional or removal examination. Confirm the current amount and transaction-specific instructions in LERIS before payment.

Before submitting
- Check whether your school record falls under PRC's Special Order exception for autonomous, deregulated, state, or accredited institutions.
- Bring the original and photocopy forms requested by PRC and follow the active LERIS appointment instructions.
- Use PRC's current list as the controlling checklist when an older reviewer guide differs.
