Qualifications stated by the Nursing Board
The Board's Resolution No. 02 s. 2022 quotes the Nursing Act qualifications: citizenship or reciprocity, good moral character, and a Bachelor's Degree in Nursing from a properly recognized college or university.
These are qualification statements from the cited Board resolution. The resolution also says the Board may make reasonable future adjustments to documentary requirements.

First-timer documents listed by PRC
The 2022 Board resolution states that repeaters have no additional documentary requirements in that resolution, subject to payment of the examination fees. PRC's live checklist and LERIS instructions remain the controlling operational references.
- PSA birth certificate.
- PSA marriage certificate or contract for a married female applicant, when applicable.
- Transcript of Records with a scanned picture and the remark “For Board Examination Purposes.”
- Notarized Certificate of Undertaking signed by the Dean and Registrar.

Confirm school and document details before submission
Check that the degree and school information on the Transcript of Records is legible and that the required purpose remark appears. Keep the notarized undertaking and any regional filing instructions with the application record.
